A Girl with Tall Stature-Is She Normal or Abnormal?

A 10-year-old girl was admitted to our hospital for the investigation of tall stature (168.6cm, +4.6SD). Breasts were well developed (Tanner 4), and pubic hair was present (Tanner 3). Menarche was observed at 9 years old. Bone age was 14 years and no acromegalic change was observed. Brain MRI revealed swelling of the anterior pituitary gland, which was enhanced by Gd. Daily profile of GH secretion was normal, and no excessive secretion of GH was found by several GH provocation tests. Loading of L-dopa resulted in a slight decrease of GH level. With the difficulty of ruling out pituitary adenoma, we decided to use somatostatin analogue, which sppressed nocturnal GH secretion and caused abdominal pain, followed by the cessation of somatostatin analogue in 1 month. Serial MRI revealed no evidence of pituitary tumor. Her height increased by only 3cm in the recent 10 months. It was concluded that her tall stature resulted from advanced pubertal progression.
