A Case of Juvenile Prolactinoma with a Chief Complaint of Short Stature

There have been very few reports on prolactin-secreting pituitary adenomas in childhood. We report here such a case of Prolactinoma (PRL-Oma: CA, 16Yr; BA, 11.5Yr), a male with short stature (HT 146cm, -3.8SDS) in a pre-pubertal state. Cranial MRI showed a pituitary tumor. Thyroid and adrenal reserve were normal, but plasma PRL was high (150μg/L), so this tumor was diagnosed as PRL-oma. The GH response to GHRH, ATT, L-dopa and plasma IGF-I were very low, and the plasma levels of LH and FSH were redueced. From his growth curve the development of PRL-oma was considered to have begun at least 6 years before. After starting bromocriptine therapy, GH reserve, IGF-I, and plasma teststerone levels are increasing concomitant with the tumor shrinkage. Special interest is focussed on the relation between the improvements of growth velocity and hormonal levels during the treatment.
