FUNDAMENTAL AND CLINICAL STUDIES ON CEFPIRAMIDE (SM-1652) IN THE SURGICAL FIELD

Fundamental and clinical studies on cefpiramide (CPM, SM-1652) was performed and following results were obtained.<BR>1) Antibacterial activities<BR>The <I>in vitro</I> antimicrobial activity of cefpiramide was compared with those of cefazolin (CEZ) and cefoperazone (CPZ) against 53 strains of <I>E. coli, Klebsiella</I> and <I>P. aeruginosa</I>. Cefpiramide had the same activity against both <I>E. coli</I> and <I>Klebsiella</I> as CEZ. Although, cefpiramide had more dominant activity against <I>P. aeruginosa</I> than CPZ with the peak MIC level of 3.2μg/ml.<BR>2) Absorption and excretion In 8 patients with biliary tract disorders, intravenous administration of cefpiramide at a dose of 0.5 or 1.0g yielded high level of bile concentration and the drug was recovered in bile with a maximum recovery rate of 40.6%. And biological half-life of the compound was estimated to be 9 hours at a dose of 1.0g. Main excretion route of cefpiramide was considered through biliary tract.<BR>3) Clinical results<BR>Cefpiramide was administered to 22 cases with surgical infections and clinical results were excellent in 5 cases, good in 9, fair in 4, poor in 2 and unknown in 2. As adverse reaction, slight elevation of S-GPT could be found in one case.
