A case of mixed germ cell tumor of the ovary composed mainly of choriocarcinoma.

We report a case of malignant mixed germ cell tumor of the ovary, which was composed of choriocarcinoma, embryonal carcinoma, and mature teratoma. A 24-year-old, gravida 4 para 1 woman was found to have a left adnexal mass. Since serum tumor markers, hCG, AFP, and CA19-9 were elevated, the tumor was suspected to be an ovarian mixed germ cell tumor. Exploratory laparotomy revealed a solid tumor of the left ovary with no apparent sign of peritoneal dissemination, and left salpingo-oophorectomy was performed. After the surgery, she received 6 courses of MAC combination chemotherapy, and is well without recurrence. Histopathological examination showed that the main component of the tumor was choriocarcinoma, being associated with embryonal carcinoma and mature teratoma. Immunohistochemically, hCG was localized in choriocarcinoma cells, and AFP was expressed in embryonal carcinoma cells. Clinical management of mixed germ cell tumors of the ovary is discussed.
