This retrospective study was undertaken to elucidate whether or not the mode of delivery affect the neonatal outcome of the second twin in the vertex-breech presentation in comparing to the vertex-vertex presentation. The outcome of 26 breech second twins weighing over 1500gm delivered vaginally were compared with 38 twins delivered by cesarean sections.<BR>No statistically significant differences in 5-minute Apgar score and umbilical vein pH were found between two groups, except that the 1-minute Apgar score was lower in the breech group delivered vaginally (P <0. 01). Among transvaginally delivered breech second twins, the 1-minute Apgar score was significantly lower when the second twin was heavier than the first twin. In selecting of the mode of delivery of vertex-breech twin, the dominance and discordance of body weight of twins must be taken into consideration for the better prognosis.
