Nipple stimulation contraction stress test: Clinical usefulness and the absence of the change of plasma oxytocin levels.

Nipple stimulation contraction stress test (NS-CST) was performed on twenty pregnant women with high risk, and the change in plasma oxytocin (OT) level during NS-CST was examined. Eighteen of 20 patients achieved adequate uterine contraction by nipple stimulation (responders), while the remaining two did not. Eleven of the 18 responders had no deceleration (negative NS-CST) and four had late deceleration (positive). The other three had exaggerated uterine contraction (hyperstimulation) without deceleration. Three of the four patients in the positive NS-CST group delivered by elective cesarean sections and the mean Apgar score of the neonates was significantly lower than that in the negative group. NS-CST was useful for assessing fetal well being or deciding the management in high risk pregnancy.<BR>The basal plasma OT level before NS-CST was 1.25±0.13 pg/ml, and the mean stimulated OT level during contraction (in responders) was 1.30±0.20 pg/ml. There was no significant increase in plasma OT. Antepartum uterine contraction by nipple stimulation was considered not to be mediated by OT.
