The first case of Acardia was reported by Benedetti in 1533. This is a rare anomaly occurring once in 34, 600 deliveries and in about 1% of monozygotic twin pregnancies.<BR>One of our patients, a 24-year-old primipara woman had sudden early abdominal enlargement. An ultrasound examination performed at 26 weeks' of gestation revealed one normal fetus and a hydropic mass with a few bones and no heart. The presence of the normal fetus with breech presentation and the mass with a few bones were confirmed by amniofetography. These findings were consistent with acardiac anomaly.<BR>An emergency caesarean section was performed when spontaneous premature labor began at 31 weeks' of gestation. A 1, 690g male infant with an Apgar score of 8 and a 1, 342g acardiac twin were delivered with some difficulty.<BR>The postoperative course was unremarkable. The premature infant was discharged after 41 days, when he weighted 3, 082g.<BR>With ultrasonography and amniofetography it was possible to delineate the features of the acardiac twin well before delivery.
