Histochemical Studies on Trophoblastic Neoplasia

In an attempt to predict the likelihood of malignant sequelae following evacuation of hydatidiform mole, cellular characteristics of trophoblastic neoplasia were histochemically analyzed by the categories of cytotrophoblast and syncytiotrophoblast in term of various enzymatic activities.Histochemical observations were made on the slices prepared from specimen of normal villi, hydatidiform mole, destructive mole and choriocarcinoma for their activities of glycolitic enzyme (LDH, G-6-PDH), steroidogenic enzyme (3β-HSD, 17β- HSD), alkaline phosphatase and acid phosphatase. The following results were drawn:1) Enzymatic activities in trophoblast of the hydatidiform mole were compared with those of normal villi at the corresponding period of gestation. In cytotrophoblast, activities of LDH and G-6-PDH were elevated in hydatidiform mole but 17β- HSD activity was reduced. In syncytiotrophoblast, activities of LDH, G-6-PDH, 3β-HSD and alkaline phophatase were intensified in hydatidiform mole. Meanwhile, 17β-HSD activity was decreased and acid phosphatase did not show any definite tendency in hydatidiform mole.2) LDH activity was recognized stronger in cytotrophoblast of normal villi than in syncytiotrophoblast, while the activity in hydatidiform mole, it was demonstrated strongly in syncytiotroph oblast rather than cytotrophoblast.3) In case in which sequential malignancy followed to the evacuation of hydatidiform mole, acid phosphatase activity was noticed to decrease in syncytiotrophoblast.4) In destructive mole, activities of LDH, G-6-PDH and 3β-HSD were enhanced in cytotrophoblast as compared with those in the hydatidiform mole, and activities of LDH, G-6-PDH, 3β-HSD and alkaline phosphatase were intensified in syncytiotrophoblast. 17β-HSD in the two trophoblastic elements of destructive mole was less active than those of hydatidiform mole.5) In choriocarcinoma, LDH activity in cytotrophoblast was comparable to that in destructive mole. However, activities of G-6-PDH, 3β-HSD, 17β-HSD andd alkaline phosphatase were lower than those in destructive mole.
