A 57-year-old woman visited Women's Clinic of the Osaka Medical College. She had the enlarged uterus, which was woman's fist in size, though with no palpable node of myoma. Hysteroscopy noticed protrusions into the uterine cavity, and diagnosed them as submucous leiomyomas. Subsequent aspiration smear, however, was class 5, and endometrial biopsy verified malignant neplasm. A part of the hysterectomized specimen revealed uterine leiomyosarcoma in succession with benign myomatous structure. One month after the operation, the patient was revealed metastasis by a chest film. In spite of additional chemotherapy, she died of brain metastasis.Generally, preoperative diagnosis of uterine leiomyosarcoma may be very difficult, but it is sure that a series of hysteroscopy, asperation smear and endometrial biopsy is available for it so far as the sarcoma develops into the uterine cavity.
