Acute Intracranial Hematoma in Infants with Bleeding Tendency:Report of Two Cases

• Two cases of infantil intracranial hematomas with a hemorrhagic tendency were experinced.The hematomas were removed under control of hemorrhagic tendency and postoperative courses were uneventful in both cases.<BR>Case I was an eightmonth-old male baby with acute subdural hematoma. Hematologic examination revealed hemophilia A. The hematoma was removed by aspiration under administration of a factor Vifi preparation. Postoperative course was satisfactry.<BR>Case II was a 47-day-old female baby. She was fed with breast milk. She was admitted to the pediatric ward of our hospital because of vomiting and bloody stool. During hospitalization she had severe convulsion, so she was transfered to our ward of neurosurgey.The diagnosis was made as acute subdural and intracerebral hematoma with hemorrhagic tendency possibly due to vitamin K deficiency. Since the bleeding time and prothrombin time were normalized by administration of vitamin K, craniotomy was performed under administration of vitamin K, and the hematoma was removed successfully. After operation, she has been growing favorably with mild right hemiparesis.<BR>Surgical treatment has heen improved of intracranial hematomas due to hemophilia A by using of a facter VIII preparation. In the case of hemorrhagic tendency due to vitamin K deficiency, the administration of vitamin K would be effective, and so the carefull hematological examination should be performed.At the present surgical procedures would be safty done in the case of vitamin K deficency and hemophilia A because of the development of hemtological examinations and prophylactic treatments.
