APPROACH OF THE CRANIOFACIAL REGION USING A DISMASKING FLAP

The "dismasking flap" is a cranio-orbito-facial degloving technique employing circum-palpebra with or without piriform margin incisions, and it presents a wide surgical fields under direct view. With use of the dismasking flap, it was possible to completely resect tumors of the nasal cavity extending the orbit, the maxilla, and the anterior cranial base. The dismasking flap, is very useful for craniofacial tumor especially for malignant tumors, for severe complex fracture of craniofacial bones, and to approach the middle cranial base. This flap is retracted far way down so that there is little traction on the facial nerve branches in the flap, and the middle cranial base comes into view with little compression on the brain.Thirteen patients with craniofacial tumors or complex fractures of craniofacial bones underwent use of the dismasking flap technique since 1993. There were no perioperative major complications. In minor complications, there were temporary lagophthalmos, blephaloptosis, disturbance of the supra-orbital nerve, and palsy of orbicularis oculi muscle, but there were no permanent complications in any cases. In this paper, we described the procedure of the dismasking flap, its postoperative course, and its complication.
