DELINEATION OF SURGICAL MARGINS FOR TONGUE CARCINOMA USING INTRAORAL ULTRASONOGRAPHY

This paper describes a new method to delineate surgical for tongue carcinoma using ultrasonography.Materials and methodsBetween 1994 and 2000, 75 patients with tongue carcinoma (T1N0M0 39 cases, T2N0M0 36 cases) underwent partial glossectomy. The surgical margin was delineated with our new method. All the cases were followed up for at least one year. Clinical and pathological evaluation of these cases was conducted.The method to delineate the surgical margin1. Preoperative examination using ultrasonography1) A #23G needle was inserted at the midline of the tongue, as on index. The distance from the mucosal surface to the index needle was measured ultrasonically (m).2) The tumor thickness was measured vertically from the mucosal surface to the maximal thickness of the tumor (d).2. Delineation of the surgical margin in the operating room1) The distance from the mucosal surface to the midline was measured with a scale (M).2) The tumor thickness (D) was corrected as follows: D=d×M/m3) The deep margin was delineated to be 10mm deeper than the tumor thickness (D).ResultsIn all cases, there were no tumor cells at the surgical margins, histologically. Only two cases developed local recurrence; however, they occurred from the mucosal surface.ConclusionOur results demonstrated that our new method to delineate the of surgical margin is useful for partial glossectomy.
