A CLINICAL STUDY ON LOCAL RECURRENCE AND PULMONARY METASTASIS OF ADENOID CYSTIC CARCINOMA OF THE HEAD AND NECK

A clinical study was made on local recurrence and pulmonary metastasis in 32 patients with adenoid cystic carcinoma (ACC) of the head and neck. The results were as follows:1) The cumulative local control rate at 5 and 10 years were 28% and 60%, respectively. Local recurrence occurred in many patients regardless of T stage, histologic pattern or surgical margin findings, when observed for a long period.2) The cumulative pulmonary metastasis rate st 5 and 10 years were 70% and 100%, respectively. Most patients showed pulmonary metastasis when observed for a long period.3) Tumor doubling time of the metastatic ACC in the lung was extremely long in comparison with that of other malignant neoplasms. In most cases, pulmonary metastasis probably occurred long time before the first visit.4) There was no relationship between local control status and the duration of survival in those who died due to the tumor. These facts indicated that control of distant metastasis was important for improvement of survival.
