The relative accuracy of fine-needle aspiration (FNA) and frozen-section (FS) in the diagnosis of major salivary gland tumors was examined. Fifty-one cases of major salivary gland tumors treated in our hospital from 1980 to 1998 were used in the study. The diagnostic sensitivity of FNA for malignant lesions was 70%, the specifity 93%, overall accuracy 82%, and the diagnostic accuracy for histologic diagnosis 43%.Thirty-one of 51 tumors were diagnosed by FNA and FS. Sixteen (48%) of 31 of these were correctly diagnosed by FNA, and 28 (85%) of 31 by FS. FS supplemented the fine-needle aspiration diagnosis in 12 cases.Mucoepidermoid carcinoma (low grade malignancy) and acinic cell carcinoma gave the most difficulty to both FNA and FS.
