High dose rate (HDR) interstitial radiation can eliminate the radiation exposure for the medical staffs. Patients can be treated without isolation in the highly shielded room. According to these benefits, HDR brachytherapy can be applied for the patients with complications or aged patients. From June 1991, we started the Phase I/II study of HDR brachytherapy for head and neck cancer, and decided the radiation dose of 60Gy/10 fractions /1 week as the standard schedule for the oral cancer without external radiation. In this paper we presented the treatment results of the HDR brachytherapy for head and neck cancer.From April 1992 through March 1995, we carried out the randomized trial of HDR and low dose rate (LDR) brachytherapy for the mobile tongue. Among 38 eligible patients, 5-year local control rates of HDR and LDR groups are 94% and 90%, respectively.From January 1992 through March 1995, 11 patients with tongue cancer patients were treated with 30 to 40Gy of external radiation and 48 to 54Gy of HDR brachytherapy. Treatment results of these patients are compared with the historical control of LDR group. Four-year local control of HDR group is 81% and 5-year local control of LOR group is 74%.From January 1992 through March 1996, sixteen patients with cancer of floor of the mouth were treated with HDR brachytherapy, and from January 1980 through March 1996, 41 patients were treated with 198Au grains. Five-year local control rate of HDR group is 94%, and these of T1 and T2 patients of LDR groups are 76% and 60%, respectively.Treatment results of HDR brachytherapy for head and neck cancer are higher or the same as those of LDR brachytherapy. HDR brachytherapy can be an alternative treatment for LDR brachytherapy.
