Free jejunal transfer has gained wide acceptance as an excellent means of pharyngoesophageal reconstruction after ablative surgery because of its many advantages. The jejunum is well vascularized, facilitating good wound healing, with a low incidence of anastomotic insufficiency, fistula formation and stricture. Donor site morbidity is minimal. The diameter of the jejunum generally matches that of the esophagostoma and it is easy to harvest.However, if the cancer invades up to the oropharynx, the resultant pharyngostoma becomes by far larger than the diameter of the jejunum, leading to postoperative difficulty in swallowing. We, therefore, performed free colon transfer to the cases with a large pharyngostoma and obtained good results. The colon has a larger diameter, a longer vascular pedicle and less perstaltic movement than the jejunum. It also can provide a longer straight intestinal tube.We describe our operative procedures of these two methods.
