A 70-year-old woman presented with pain in the right hypochondrium and jaundice. Her general condition was poor for her age. MRCP revealed bile duct carcinoma associated with pancreaticobiliary maljunction without bile duct dilatation. Bile duct resection was performed considering her general condition. Histopathological examination showed advanced cancer and the presence of residual cancer. Based on the results of this case, it is surggested that constructive bile duct resection should be performed for cases of cancer with pancreaticobiliary maljunction without bile duct dilatation.
