Effect of Ibudilast on Experimental Diabetic Neuropathy.

3-Isobutyry1-2-isopropylpyrazolo [1, 5-a] pyridine (ibudilast) has been considered to ameliorate atherosclerosis and to relax vascular smooth muscle. Since amelioration of deranged vascular factors as well as metabolic factors has been considered to be needed in the treatment of diabetic neuropathy, we examined the effects of ibudilast on motor nerve conduction velocity (MNCV) and nerve blood flow (NBF) in the sciatic nerves of rats with streptozocin (STZ) induced diabetes. The teased fiber method was used for pathologic study. A control group, a high dose ibudilast (10 mg/ kg/day) group, an untreated diabetic group, and diabetic groups treated with ibudilast (low-dose ibudilast: 1 mg/kg/day and high-dose ibudilast: 10 mg/kg/day) were tested for 8 weeks (n=7 each). Ibudilast was given orally. After 8 weeks of treatment MNCV and NBF were significantly lower in the untreated diabetic group. In the ibudilast-treated diabetic groups MNCV was significantly better than in the untreated diabetic group, and NBF had become normal. The effects occurred in a dose-dependent manner. The preventive effect of ibudilast on diabetic neuropathy was also significant in the teased fiber study. Ibudilast alone did not affect fasting blood glucose, MNCV or NBF levels.<BR>These results suggest that ibudilast may play a preventive role in the treatment of diabetic neuropathy.
