Diabetic gangrene is known to be related to abnormalities of the coagulation and fibrinolytic system. Gangrene is grouped into two types i. e., with severe ischemic change in the limbs (atherogenic type) and with severe neuropathy (neurogenic type). Tissue plasminogen activator-plasminogen activator inhibitor complex (tPA-PAI complex) is a new marker of fibrinolysis. In the present-study, the influence of the fibrinolytic system on diabetic gangrene was investigated.<BR>Thrombin antithrombin III-complex (TAT), tissue-plasminogen activator (tPA), plasminogen activator inhibitor 1 (PAI1), α2-plasmin inhibitor-plasmin complex (PIC) and tPA-PAI complex concentrations were determined by enzyme immunoassay in 27 diabetic patients with gangrene (16 atherogenic type, 11 neurogenic type), 22 diabetic patients without gangrene and 10 healthy controls. TAT and tPA were significantly higher in the diabetic group, with or without gangrene, than in the healthy controls, and there were no significant differences in the results of PAI-1 among these three groups. NC and tPA-PAI complex were significantly higher in diabetic gangrene than in the diabetic group without gangrene.<BR>The results of this study indicate that fibrinolysis is accelerated in patients with diabetic gangrene.
