The time course of the prevalence of islet cell antibodies (ICA) was studied in 88 children with insulin-dependent diabetes mellitus (IDDM), of whom 55 had abrupt onset mode (abrupt IDDM), and 33 had slow onset mode detected by urine glucose screening (slow IDDM). This time course was compared with that of 52 children with non-insulin-dependent diabetes mellitus (NIDDM).<BR>The prevalence of ICA in IDDM patients was significantly higher than that in NIDDM patients at diagnosis (67%vs 9%, p<0.01). Among the IDDM patients, abrupt IDDM patients initially had a significantly higher prevalence of ICA than slow IDDM patients (50%vs 78%, p<0.05). However, ICA were more freqently detected in slow IDDM patients after a duration of three years or more. The decline of the prevalence of ICA in slow IDDM was less marked than that in abrupt IDDM during the course of the disease. On the other hand, the patients with prolonged persistence of ICA tended to have persistent anti-thyroid antibodies and relatively high serum C-peptide.<BR>From these findings, it is probable that in slow IDDM patients there was slowly progressive destruction of pancreatic B-cells by autoimmune reactions, and that progression in the early phase of diabetes in slow IDDM patients was relatively mild.
