Changes in plasma β-thromboglobulin (β-TG) and tissue plasminogen activator (t-PA) concentrations after insulin injection were measured in 10 non-diabetics (non-DM) and 31 non-insulin-dependent diabetics (DM) without (DM-1, n=16) and with (DM-2, n=15) macro-and/or microvascular complications.(1) Basal plasma β-TG concentrations in DM-2 subjects were 3.7 times those in non-DM subjects (p>0.001).(2) Plasma β-TG concentrations during an acute decrease in glucose concentration in the DM-2 group were significantly higher than in both the non-DMandDM-1 groups. The magnitude of the increase in plasma β-TG concentration in the DM-2 group was significantly higher than in the other groups.(3) The increases in plasma β-TG concentration were positively correlated with the decreases in plasma glucose concentration (r=0.40, p<0.01).(4) Plasma t-PA concentrations did not change during an insulin-induced decrease in plasma glucose levels in either the non-DM or DM group.<BR>These results indicate that in vivo platelet aggregation was increased during an acute decrease in plasma glucose levels, depending on the magnitude of the decrease and the existence of diabetic vascular complications.
