Application of a Systematic Manual for Training Swallowing Disorders to Postoperative Oral Cancer Patients.

To facilitate recovery of swallowing function after oral cancer surgery, a new systematic training manual for swallowing therapy was developed. We used this manual to train two patients with different areas of resection, who both showed aspiration and severe penetration on videofluorography (VF). Patient 1 had undergone a subtotal glossectomy, while patient 2 had received a hemi glossectomy.<BR>Training items were chosen based on the results of our clinical assessment test series, including items showing a " negative" assessment in the training program. Patient 1 had " negative" assessments for " coughing", " neck stretching", " cheek/lip", " tongue", " laryngeal elevation", and " articulation". Patient 2had " negative" assessments for " neck stretching", " tongue", " laryngeal elevation", and " articulation".<BR>It took a long time for patient 1 to achieve " back tongue elevation", and for patient 2 to achieve " put the tongue tip at an angle to the mouth". But after 6 months of training, the swallowing function of both patients recovered.<BR>The results indicated that our training manual for swallowing therapy was useful and efficient for patients with dysphasia after oral cancer surgery, considering the resected area and severity of dysfunction.
