Analysis of Seven Cases of Basal Cell Nevus Syndrome.

Basal cell nevus syndrome is a dominant chromosomal hereditary disease whose predominant symptoms include multiple basal cell tumors of the skin, multiple cysts of the jaw and ectopic calcification. The multiple cysts of the jaw often provide an opportunity to diagnose this syndrome. Since jaw cysts tend to recur in many places even after enucleation, correct diagnosis and adequate treatment of these cysts by oral surgeon are important. Over the last 17 years, seven patients suffering from this syndrome have been examined at our clinic. These patients as well as the treatment methods used are reported.<BR>There were five male and two female patients, and three of the seven patients were related. In all patients, typical faces, multiple jaw cysts, bridged sella turcica and calcified falx cerebri were found. However, cutaneous symptoms such as palmer pit or nevus were found in only three patients. The generation area is with in the area of the tooth germ. It was suggested that cyst generation was related to tooth eruption.<BR>The multiple jaw cysts were treated by enucleation in many cases. However, in younger patients with extremely large cysts, marsupialization was performed to carry out secondary enucleation. Small cysts were closed, but large ones were left open. Further, after completely removing the bone from the enucleation cavities, particulate cancellous bone and marrow (PCBM) graft was performed. So far, there has been no recurrence. Since keratocysts are likely to recur and to have the symptoms of basal cell carcinoma, although the frequency is low in Japan, a long-term follow-up study is important.
