A 61-year-old female was referred to our Oral and Maxillofacial Surgery Department for evaluation of swelling of the gingiva adjacent to the left lower first molar. Since microscopic examination revealed a dense aggregate of plasma cells, myeloma was suspected. However, after thorough investigation, final diagnosis as a plasma cell granuloma could be made.<BR>Microscopically, the plasma cells showed no cytologic abnormality and were accompanied by other inflammatory cells. Russel bodies were seen. In addition, the immunoperoxidase technique showed polyclonal population of plasma cells.<BR>Treatment consisted of total resection of the lesion including the alveolar bone. No signs of recurrence have appeared 9 years postoperatively.
