Plasma prostaglandin I2 regenerating activity in diabetic patients.

It has been reported that normal plasma (PPP) stimulates prostacyclin (PGI2) production by aortic endothelial cells. Since PGI2 has potent antiaggregatory and vasodilating activities, this plasma PGI2-regenerating activity could be of etiological and clinical importance in thrombotic disorders. We examined it in diabetic patients by the modified method of Remuzzi et al. PGI2-like activity was assayed by the inhibitory effect on ADP-induced platelet aggregation of citrated plasma which had been incubated with the "exhausted" rat aorta for 1h at 37°C. Control PPP obtained from 27 normal volunteers stimulated the exhausted rat aorta to produce 0.84±0.17 (M±SD)ng/mg aorta/h of PGI2-like activity, while PPP from 20 diabetic patients showed a significantly decreased activity (0.48±0.30) as compared with normal one (p<0.001). Because the reduced PGI2-regenerating activity was improved in 6 of 9 patients during appropriate treatment, this reduced plasma activity appeared to be reversible and related to the degree of diabetic control. Patients with diabetic retinopathy had a significant decrease (p<0.05) in their plasma PGI2-regenerating activity (0.30±0.20, n=10) compared with those without retinopathy (0.66±0.26, n=10). Thus, the decrease in plasma PGI2-regenerating activity of patients with diabetes mellitus may play a pathogenic role in the development of the microvascular and thrombotic complications frequently seen in this disorder.
