Preoperative Prognostic Factors Associated with Colorectal Perforation

We conducted a clinical investigation of 58 cases with colorectal perforation who underwent an emergency operationat our hospital from March 1999 to March 2005. The mean age of the patients was 72.2. Nine cases died giving amortality ratio of 15.5%. We also analyzed possible preoperative prognostic factors. The results showed that anextended period between the onset of the clinical symptoms and the operation, a preoperatively positive blood culture, a low preoperative WBC count and a high preoperative APACHE-II (acute physiologic and chronic health evaluation II) score were all related to a poor prognosis. The mortality rate in patients with these factors exceeded 50%. Earlydiagnosis and operation are important in order to assure the survival of these severe cases, and vigorous antisepsisregimens are essential, particularly in the case of patients with these poor prognostic factors mentioned above.
