Separation and preservation of umbilical hematopoietic stem cells prepared in the Nara cord blood subbank.

For hematopoietic stem cell transplantation, the collecting method of umbilical cord blood, the separation method of hematopoietic stem cells from the umbilical cord blood, the preservation method and the recovery of hematopoietic stem cell activities after thawing cryopreserved cord blood cells, and engraftment of the cord blood stem cell transfusion were studied. In the third stage of labor in a normal term pregnancy, a specially-prepared bag connecting tube equipped with a needle is very useful for collection of a large quantity of umbilical cord blood and hematopoietic stem cells. In this method, separated hematopoietic stem cells are sufficient for hematopoietic stem cell transplantation to children. The collected volume of the cord blood was significantly correlated with the cell number of stored CFU-GM or CD34 positive cells. Processing time for the separation of hematopoietic stem cells after the collection of the cord blood was inversely correlated with the stored cell nember of CFU-GM. After thawing cord blood cells that had been cryopreserved for 30 months such as CFU-GM, BFU-E, and CFU-Mix cells, the recovery ratio of viable cells was more than 80%. Cord blood registered to the Kinki cord blood bank accounted for 50.2% of the collected cord blood, and 60% of the blood samples were not unregistered because of insufficient volume for the separation of hematopoietic stem cells. By the time of this study, 8 samples from cord blood samples registered in the Nara subbank had been used for hematopoietic stem cell transplantation. Five of them resulted in successful engraftment, but 3 were rejected, and the reasons for the unsuccessful results are unknown. Negative results for Wassermann test for syphilis, TPHA test, hepatitis B surface antigen, hepatitis C antibody, HIV antibody, HTLV-1 antibody, and DNA of cytomegalovirus were obtained from all collected cord blood samples. Althogh the collected volume of the cord blood from the term placenta is small in our study, the number of hematopoietic stem cells is sufficient for hematopoietic stem cell transplantation to children.
