Stillbirth associated with diabetes mellitus and Epstein-Barr virus infection ; A case report.

Diabetes mellitus during pregnancy is not rare, and it is well known that viral infection, autoimmunity and certain HLA antigens are closely related to the onset of insulin dependent diabetes mellitus.A 24-year-old primipara complaining of lower abdominal pain and nausea in the 29th week of pregnancy was referred to our hospital because of intrauterine fetal death. For vaginal delivery of the fetus, prostaglandin F2-alpha was started by intravenous infusion. However, the patient lost consciousness and became comatose. An emergency cesarean section was carried out, and a stillborn male infant weighing 970gm was removed. After the cesarean section, intravenous insulin infusion was given because laboratory tests showed ketoacidosis.During her hospital stay, her IgG antibody to viral capsid antigen of Epstein-Barr virus (EBV) increased from 1: 40 to 1: 640, and IgG antibody to early antigen of EBV increased from 1: 20 to 1: 80, indicating EBV infection. Anti-DNA antibody, thyroid tests and microsome tests were negative. Her HLA typing was a phenotype of AW 24, A(-), B 7, BW 46, CW 1, CW 3, DR 1, DRW 9, MT 1, MT 3, MT 4.
