視野欠損のための長期連続的bromocriptine療法を行ったprolactinoma合併妊娠の1例

A pregnancy occurred in a patient with prolactinoma after treatment with Bromocriptine and HMG-HCG. At 8 weeks of gestation, she complained of slight headache and visual disturbance. Ophthalmologic examination disclosed visual field defects in both eyes and examination with CT scanner showed suprasellar enlargement of the tumor.Treatment with 5mg Bromocriptine/day was started resulting in rapid and marked improvement of the symptoms and decrease in size of the tumor. The same dose of the drug was continued throughout the period of gestation, because reduction of the dose was followed by recurrence of the symptoms.The serum PRL dropped rapidly after the initiation of the treatment, however, it gradually increased toward the end of pregnancy in spite of Bromocriptine administration. But, neither visual symptoms nor enlargement of the tumor was noted thereafter.The delivery was uncomplicated and a normal female child (2.8kg) was born at 39 weeks of gestation. The serum PRL level in the child was very low at birth and increased in the following months.This case study suggests that Bromocriptine is effective in reducing the size of prolactinoma even in pregnancy and that it crosses the placenta and affects the fetal pituitary suppressing PRL production.
