Localized osteomyelitis of the metacarpus IV (Mc IV) was successfully treated surgically in a steer. The patient showed severe lameness at the affected limb with a moderate swelling and a fistula with purulent discharge. Radiography revealed a sharply delineated bone piece (sequestrum) with a radiolucent zone at the Mc IV. In addition, periosteal new bone formation, the thickened cortex and increased medullary density were observed. Sequestrectomy was performed under xylazine sedation and local anesthesia. After the removing, the surgical wound was kept open for irrigation and the casting external fixation was employed. Swelling of the limb and the lameness diminished in approximately 3 weeks.
