Comparison of the Treatment Results of Radiation Therapy Combined with Bleomycin, Peplomycin, or Ciplatin for Oral Cancer.

Between 1972 and 1989, 723 patients with squamous cell carcinoma of the oral cavity were treated with initial radiation therapy. The primary sites included oral tongue 230, floor of mouth 141, buccal mucosa 94, gingiva 258. Four-hundreds and nine (56%) patients had T1 or T2 tumors, and 352 (48%) had cervical lymph node metastasis. Of these, 289 patients were treated by radiation (24-30Gy/3 weeks) combined with bleomycin (BLM) (90mg/3 weeks) and 241 with peplomycin (PEP) (45-60mg/3 weeks). Thirty-four were treated by radiation (40Gy/4 weeks or 50Gy/5 weeks) combined with cisplatin (CDDP) (6mg/body daily, with total of 120mg/4 weeks, or 150mg/5 weeks).All patients of BLM and PEP group experienced severe mucositis. However, only 40% of CDDP group experienced moderate mucositis, and none had severe one. These mucositis was disappeared withn 2-3 weeks after the end of treatment.The total response rate (CR+PR) was 94%, with complete response (CR) of 42% for BLM group, 35% for PEP group, and 56% for CDDP group. CR rates of radiation alone were 11% for 30-49Gy group and 37% 50-80Gy group. Higher CR rates were obtained in patients with T1 or T2 cases compared to T3 or T4. In patients with carcinoma of the gingiva (65%), and the cheek mucosa (44%), obtained higher CR rates comparing with carcinoma of the floor of the mouth (25%), and the tongue (20%).Most of patients with partial response had subsequent surgery or brachytherapy. However, 64% of patients with CR, were followed without any further treatment. Two-year survival rates were 81% for CR cases, and 53% for PR cases. Survival rates without surgery were 41% for CR cases, and 18% for PR cases. No patients developed late complications.
