Morphologic Changes in the Peripheral Nerves in Inflammatory Cutaneous Lesions, especially in Leprosy

The peripheral nerves of the skin in leprosy were studied morphologically using chiefly Suzuki's silver impregnation method. Specimens were taken from 24 patients, including 7 cases of indeterminate type, 8 of lepromatous type, 7 of tuberculoid type and 2 of borderline type.<BR>Other cutaneous granulomas such as tuberculosis cutis, xanthoma, Boeck's sarcoid etc. were studied as controls.<BR>The results were as follows: Degenerating process of the peripheral nerves depended on different types of leprous lesions. In the indeterminate type, Henle's sheath was kept almost intact while neurofibrils were degenerated, showing slightly argylophilic balloon-like swelling in some of them. This was considered as a unique, hitheto-unrecognized change in leprosy. In the lepromatous type, neurofibrils were scarce or absent, though Henle's and Schwann's sheaths and axoplasm were kept relatively intact. In the tuberculoid type, all the components of the nerve showed a tendency towards degeneration. In the borderline type, features seen in both tuberculoid and lepromatous types were observed.
