Usefulness of Indirect Immunofluorescence Using NaCl-separated Skin and Immunoblotting Using BP180 Recombinant Fusion Proteins for the Diagnosis of Pemphigoid.

In order to know the useful methods for the definite diagnosis of pemphigoid, we examined 117 sera from patients diagnosed as pemphigoid by clinical features, histology, and routine direct and indirect immunofluorescence (IF). In this study, we carried out indirect IF using salt-split skin as substrates. Two out of 117 sera showed dermal pattern and these two sera were finally diagnosed as non-scarring inflammatory type of epidermolysis bullosa acquisita. By immunoblot analysis using glutathione-S-transferase (GST) recombinant proteins of 180kD bullous pemphigoid (BP180), 110 out of 115 sera reacted positively with these BP180-GST recombinant proteins. These results indicate that indirect IF using NaCl-separated skin and immunoblotting using BP180 recombinant proteins are useful methods for the diagnosis of pemphigoid.
