The patient was a 80-year-old male with a half palm size erosive mass on the posterior surface of the left lower leg. His daughter has similar eruption. At the age of about 50 years, a dark brown macule appeared in the posterior area of the left auricle. Subsequently, similar eruptions extended to the face, trunk, and both upper and lower extremities.<BR>In January, 1994, erosive erythema appeared on the posterior surface of the left lower leg. In January, 1995, this erythema had become elevated into a mass. He was treated at a local hospital but showed no improvement, and was referred to our department on August 23, 1995. Confluent annular and circinate eruptions were observed in the entire body including the face (about 85% of the body surface area) and multiple erythema and erosion areas to a maximum size of the thumb head at about 20 sites (Bowen disease lesions varying in size at more than 17 sites). Examination using a videomacroscope revealed follicular spinous vellus hairlike processes in the annular eruptions. Light and electron microscopic examination showed follicular cornoid lamella.
