A Case of Bullous Pemphigoid with Circulating Autoantibodies against the BP180 and Basal Cell Cytoplasm.

A 65-year-old man developed bullous skin lesions on the trunk and extremities. Biopsy specimen obtained from bullous skin lesions showed subepidermal blister formation with an infiltration of eosinophils. Direct immunofluorescence showed the depositions of IgG and C3 at the basement membrane zone. Indirect immunofluorescence using normal human skin as substrates revealed the circulating IgG autoantibodies reactive with the cytoplasm of the epidermal basal cells. Immunoblot analysis using normal human epidermal extracts demonstrated that this patient's serum contained IgG autoantibodies against the 180kD and 60kD proteins. From these findings, this patient was diagnosed as bullous pemphigoid with circulating IgG autoantibodies against the BP180 and keratin intermediate filaments of the epidermal basal cells.
