Bilateral herpes zoster developed on the trunk of a 9-year-old-boy with a history of chickenpox vaccination. He had received the vaccine at about 3 years of age and subsequently had suffered natural chickenpox infection half a year after the vaccination. To ascertain whether the strain from the lesion was the wild or the vaccine strain, we analyzed the numbers of reiterations in the sequences, R1, R2, R4, R5 and the mutation of the Pst I cleavage site on the varicella-zoster virus (VZV) genome using the polymerase chain reaction. The results suggested that the clinical strain was the wild type. It is speculated that the patient developed herpes zoster by the reactivation of the wild type strain that latently infected his ganglia after he had had the natural chickenpox infection, because he failed to gain a complete immunity to VZV from the vaccination.
