An autopsy case of Dermatofibrosarcoma Protuberans

A case of Dermatofibrosarcoma protuberans in left lower leg of a 31 year-old male is presented, and its clinical course and postmortem findings are also described.<BR>1) Case history was that the tumor in the left leg developed two years prior to admission. It failed to heal despite of the twice surgical excisions and grew to the size of a fist with ulceration. X-ray irradiation and anticancerous drugs were ineffective. After oral ingestion of THQ, the tumor apparenty diminished in size and broke down, but the patient died following generalized metastasis to the skin of breast, abdomen and viscera.<BR>2) Autopsy discolsed scattered invasion of the tumor to the skin of breast, left abdomen, myocardium, both lungs and pleurae. The primary lesion revealed histologically the proliferation of fibrous cell bundles lined in whirls, with portions of mucoid and hyaline degeneration. In metastatic foci, esp. in the myocardium, atypical patterns were noted. Such findings were suggestive of fibroma and fibrosarcoma.<BR>3) There has been no reported case on the postmortem examination of Dermatofibrosarocoma protuberans with generalized metastasis, and great emphasis is placed here to note the fact that this entity has a possiblity to metastasize with malignancy.
