A study on indications, methods and results of operative mobilization for stiff elbow joints with diverse etiologies.

Sixty-five stiff elbow joints with diverse etiologies whose ROM was less than 120° flexion and minus 40°extension, in which operative mobilization was carried out either with capsu-lotomy or -lectomy with or without synovectomy and joint debridement, or with resection arthroplasty with an interposing membrane or with total elbow replacement, were followed up and over-all results were assessed with three indicators, i.e., pain, ROM and complications such as instability.<BR>The over-all results of capsu-lotomy or-lectomy with or without joint debridement and of resection arthroplasty in ankylosed elbow joints were good. Those of capsular release or joint debridement in osteoarthritic elbow joints were unsatisfactory. Good results were also obtained with synovectomy in an RA-afflicted elbow without marked cartilage degeneration, although the progression of osteo-arthritic change did not seem to have been arrested by this procedure. There was much room for improvement in resection arthroplasty and total elbow replacement and cautious usage of elbow joints after these operations was thought advisable to maintain the postoperatively improved function.
