Patello-femoral alignment and "chondromalacia patellae".

This study aimed at clarifying how the alignment of the patello-femoral joint is related to the 'chondromalacia patellae' and what the mechanism is which causes cartilage lesions of the patella. The degree of alignment on 1242 knees of 621 cases who claimed trouble with at least one knee was classified into one of four grades according to dynamic sky-line view arthrograms. The patients age range was from 15 to 35 years. The grade distribution of all cases studied was Grade 1…62.1 %; Grade 2…25.8 %; Grade 3…6.7 %; and Grade 4…5.4 %. Clinical disorders occurred at high percentages in cases of Grade 3 and 4.<BR>The cartilage lesions on the patella were observed in 83 of 127 Grade 3 and 4 joints (65.3 %) on performing either arthroscopy or open surgery. From the observations it was concluded that cartilage lesions were caused by shearing stresses produced by the abnormal motion of the patella due to the malalignment of the patello-femoral joint.<BR>Thus it was indicated that there exists a causal relationship between the patello-femoral alignment and"chondromalacia patellae".
