From 1973 to 1985, Chiari's pelvic osteotomy was performed on 32 hips of 26 patients of five men (7 hips) and 21 women (25 hips) at our hospital. The average age at operation was 32 years (16-55 years) . Radiological findings had shown that 20 hips of 16 patients had pre-coxarthrosis or early coxarthrosis, and 12 hips of 10 patients had advanced coxarthrosis. In the operation, we cut off a major trochanter, exposed a hip joint and performed a dome-shaped osteotomy.<BR>According to the hip score of the Japanese Orthopedic Association, hips with pre-coxarthrosis and early coxarthrosis improved from 70.6 points to 83.3 points and those with advanced coxarthrosis from 62.2 points to 75.2 points. One of the former and one of the latter were aggravated, while evaluation of X-rays showed that one of the former (5%), and three of the latter (19%) were.<BR>It seems that Chiari's pelvic osteotomy is a very useful method for pre-coxarthrosis and early coxarthrosis with acetabular dysplasia and subluxation of the hip, but is somewhat less useful for advanced coxarthrosis.
