Total Knee Arthroplasty in HIV-positive Haemophilic Knee Arthropathy Patients

Total knee arthroplasty (TKA) is very effective in relieving pain and improving function in pa-tients with advanced hemophilic arthropathy.<BR>Objective: To evaluate the results of TKA in patients with HIV-positive severe hemophilia A.<BR>Patients and Methods: We evaluated 5 TKAs in 4 patients. The average length of follow-up period was 24 months. The mean age at surgery was 47 years. Factor VIII survival analyses were performed before surgery. Factor VIII supplemental therapy was aimed to maintain the level of factor VIII at 100% on the day of surgery and initial postoperative period.<BR>Results: A significant difference was observed between the preoperative and post-operative JOA knee scores (averaging 38 points before surgery, and 73 points after surgery) . In range of motion, the average extension was significantly improved from -30 degrees to -7 degrees. On the other hand, average knee flexion was not significantly improved. One complication (a hematoma) occurred in this series.<BR>Conclusion: TKA for hemophilic arthropathy with HIV-positive patients provides good results concerning the function of the knee.
