Between 2001 and 2003, 17 patients (19hips) underwent total hip replacement or hemiarthroplasty for young patients using the Mayo conservative femoral prosthesis, of a short double-tapered wedge design. The mean age of the patients was 53.9 years and mean duration of follow-up was 23.5 months (range 12-32) . Radiological analysis showed the following results: the stems were implanted in valgus alignment (mean: 3.5°) by A-P view, and anterior tilted alignment (mean: 0.6°) by lateral view. Six stems (32%) made direct contact between the lateral plane and the lateral cortex of the intramedullary canal by A-P view, 53% had unclear radiolucent lines (RLLs) less than 1 mm around the distal plane of the stem, and 26% had clear RLLs along the greater trochanter. Nevertheless, 63% of the stems had spotwelds in the middle portion of the stem and along the medial metal-fiber pads, and none of the stems had migration or thigh pain in this study.<BR>The results of our study suggest that the Mayo conservative femoral prosthesis for young patients gives favorable proximal femoral remodeling and reliable fixation rather than instability.
