Total Ankle Arthroplasty with Talocalcaneal Arthrodesis for Hindfoot Lesions

Purpose: Ankle and talocalcaneal joints are sometimes both damaged by rheumatoid arthritis (RA) at the same time, and the method of treatment for that disorder is controversial. The purpose of this study is to investigate the usefulness of total ankle arthroplasty (TAA) with talocalcaneal arthrodesis for the cases that have disorders in both ankle and talocalcaneal joints.<BR>Materials and methods; In the study group, there were 4 patients; 2 patients with both feet having this disorder and 2 patients with a single foot having this disorder. A11 patients were women aged 45 to 60 years old at the time of surgery (average, 57 years old) . The duration of therapy for RA was from 12 to 19 years (average, 15 years) and follow-up was from 8 months to 3 years and 1 month (average, 1 year and 10 months) . We used the AOFAS Ankle-Hindfoot scale for clinical evaluation, and examined X-ray findings before and after surgery.<BR>Results: The average AOFAS Ankle-Hindfoot scale reading improved from a presurgical reading of 33.0 points to a postsurgical reading of 67.2 points. On X-ray findings, we could not find any change for the worse in adjacent joints, and ascertained complete union of talocalcaneal joints in all cases. However, in one case in which cement was not used, there was loosening and sinking on the surface of the talar component.<BR>Conclusions: Because we can expect symptom improvement and preservation of hindfoot function, TAA with talocalcaneal arthrodesis is a useful procedure for the disorders in both ankle and talocalcaneal joints of RA.
