Mobile-bearing total knee arthroplasty (MBTKA) has potential advantages compared with conventional fixed-bearing TKA. By allowing free axial rotation, MBTKA might be designed with greater articular conformity without increased axial torque and linear wear rate. Axial rotation of the platform also allows self-aligning of the component rotation. Despite these theoretical advantages, the kinematics of MBTKA are similar to those observed in fixed-bearing TKA. The long term clinical results obtained with current MBTKA also are not superior, and sometimes inferior to those obtained with well-designed fixed-bearing TKA prostheses. The disadvantages of MBTKA include mechanical failure, such as bearing dislocation and breakage, and increased volumetric wear. Overall, the advantages of MBTKA have been largely theoretical, and any real benefit of this design remains to be proven by detailed kinematic analysis and long-term follow up.
