A retrospective clinical and radiographical review was performed on 13 cementless total hip arthroplasties in patients aged 49 or less, with an average follow-up of 12 years and 3 months (range, 10 to 17 years) . All arthroplasties involved the use of a Jikei cementless total hip femoral and acetabulum component, which was fixed to the bone with a macro-anchoring system. The average JOA hip score was 32.2 points preoperatively and 77.1 points at 10 years postoperatively. Ten patients (77 %) showed successful results of more than 70 points. In the radiographical analysis, femoral component subsidence was noted in every case; the average subsidence was 8.8mm, and the width of the radiolucent zone around the femoral component was 2.1 ma. An increased acetabular component angle of 7.8°, and central migration of 7.4 mm were noted on an average at 10 years follow-up. The clinical and radiological results in patients less than 49 years not differ from those in patients 60 or more.
