We retrospectively surveyed 43 ears operated on from April 2000 to March 2002 at St. Marianna University School of Medicine.
Surgery success was analyzed using the three criteria suggested by the Japan Clinical Otology Committee in 1999. The procedure was classified as successful when the hearing level 6 months after surgery met at least one of these three conditions : 1) an air-born gap of less than 15dB, 2) a gain of more than 15dB, or 3) a hearing level above 30dB. We used the autograft cartilage for a columella for ossicular reconstruction.
Success in all cases was 69.8%. In chronic otitis media, it was 87.5%. In cholesteatoma, it was 56.5%. We found no significant difference in success among canal-wall-down, canal reconstruction with mastoid obliteration and, canal-wall-up tympanoplasties. Success was 81.8% for type I tympanoplasty, 70.8% for type III with columella or interposition, and 50.0% for type N with columella or interposition.
