The clinical records of 223 patients of the middle ear diseases, excluding the malignant tumors, operated on at the Department of Otolaryngology of University of Tsukuba Hospital from March 1988 to March 1998 were reviewed. Two hundred and seventy-six ears underwent the ear surgeries, including 8 of staged surgery and 23 of revision. The majority of patients (132 ears ; 47.8%) were diagnosed as chronic otitis media with cholesteatoma, and 58, 40 and 12 ears were diagnosed as chronic otitis media without cholesteatoma, ossicular fixation or discontinuity and otosclerosis, respectively. Myringoplasty, tympanoplasty, stapes sugery, radical mastoidectomy and others were performed in 17, 202, 25, 18 and 14 ears, respectively. Columellization from the head of stapes was performed in 104 ears and from the foot-plate in 26 ears. Of the 188 ears, a trimmed tragal cartilage were used in 154 ears for the reconstruction of the ossicular chain and an artificial ossicular reconstructive prosthesis (PORP or TORP) was used in 28 ears. The overall success rate as to an improved hearing was 81.8% ; there was no significant difference between the cases of chronic otitis media with (78.4%) and without cholesteatoma (78.0%), nor between the intact canal (88.0%) and canal down (75.3%) cases operated on for the surgical treatment in chronic otitis media with cholesteatoma.
