72-year-old man was admitted with bronchogenic squamous cell carcinoma detected by sputum cytology. Bronchoscopically, a tiny nodule (measuring about 3 mm) was found at the orifice of right B<SUP>1</SUP>. Right upper lobectomy was performed. Histological findings revealed hilar lymph node involvement by the tumor which was limited to within the subcartilaginous layer of the bronchial wall. Six months after the operation, he suffered multiple spinal metastasis and died 9 months after the operation.
