Evaluation of the Effectiveness of Lung Cancer Screening in Sukumo City: An Attempt to Eliminate the Length Bias from Survival Analysis with Consideration of Screening History.

In Sukumo city, almost all lung cancer patients derive from the lung cancer registry. In order to evaluate the effectiveness of lung cancer screening, a total of 106 lung cancerpatients (40-79 years of age) who were detected in Sukumo city during the past seven year (1987-1993) were analyzed.First, we compared 46 subjects detected by the population based lung cancer screening with 60 subjects detected by other means.Concerning the screening detected cases, the detection rate was 99.9/100, 000, the standardizeddetection ratio was 1.04, the rate of clinical stage I lung cancer was 74%, the resection rate was 74%, the absolute curative resection rate was 57% and the five-year survival rate was 55%.Prognosis of lung cancer cases detected by screening was significantly better than that of those detected by other means.However, when we investigated the screening histories of all cases, there were 20 interval cases, and the outcome of which was very poor.For the purpose of eliminating length bias caused by interval cases, we compared 59 subjects participating in the screening at the preceding year of diagnosis with 47 subjects not participating in it.The five-year survival rate was 44% in the former and 16% in the latter.The median survival time of the former was 47 months which was significantly longer than that of 17 months for the latter.This may suggest that a careful lung cancer screening on an annual basis could reduce the mortality rate from lung cancer.
