Evaluation of Segmentectomy for Roentgenographically Occult Lung Cancer.

In order to preserve lung function with curative intent, we performed a total of six segmentectomies for five roentgenographically occult lung cancer patients. All bronchi were preoperatively observed and brushed. Biopsy was performed for all lesions.<BR>Whether invasion was intrabronchial or extrabronchial was determined by bronchoscopy, cytological and histopathological analysis. Three out of six lesions could not be determined as intrabronchial invasion due to lack of one or two of these three analytical findings.<BR>Surgical specimens revealed extrabronchial invasion in two of the three lesions.<BR>In our institute no lymph node metastasis has been found in over one hundred cases of roentgenographically occult lung cancer with intrabronchial invasion which were resected completely.<BR>Segmentectomy therefore can be regarded curative resection for intrabronchial invasion cases. As for extrabronchial invasion cases, however, segmentectomy has not been proved to be curative, even if the sampled lymph nodes were free from metastasis.
